We are currently looking for a motivated and well organised Care coordinator/schedular to work with our team in our Mayo office, based in Castlebar town. Free parking.
Requirements:
Excellent communication skills
Ability to work proactively and efficiently.
Good knowledge of IT processes
The responsibilities of the care coordinator include and are not limited to;
· Accept, assign, and manage referrals for new service users based on the available caregivers in a specific areas.
· Communicate effectively with families and the HSE regarding homecare support packages and coordinate services for clients
· Coordinate with clients and their representatives regarding their care needs, providing assistance as needed.
· Ensuring that all systems are being used to full capacity and that all contact with caregivers and clients are logged on our IT system
· Liase with recruitment team to facilitate ongoing training for caregivers and any other.
