We are currently recruiting for Clinical Pharmacist in our Beaumont Lodge Transitional Care Unit, Coolock, Dublin 5.
Beaumont Lodge, is a 221-bed HSE-funded Transitional Care Unit. The postholder will play a central role in ensuring safe and effective medicines management for patients transitioning from acute hospitals to sub-acute and community care.
Contract: Full Time- Permanent
Key Responsibilities
- Carry out medicines reconciliation on admission, transfer, and discharge.
- Review and optimise medication regimens with focus on polypharmacy, deprescribing, and safe prescribing.
- Provide clinical advice on dosing, interactions, and monitoring of high-risk medicines (e.g. anticoagulants, insulin, antibiotics, opioids).
- Support patient and family education on safe use of medicines on discharge.
- Participate in ward rounds and discharge planning meetings with the wider care team.
- Contribute to medication safety audits, antimicrobial stewardship, and quality improvement initiatives.
- Provide training to nursing and care staff on safe medicines administration.
Requirements
- Degree in Pharmacy (MPharm or equivalent).
- Current registration with the Pharmaceutical Society of Ireland (PSI).
- At least 1–2 years’ post-qualification experience in a hospital, primary care, or transitional care setting.
- Experience in geriatric medicine, rehabilitation, or polypharmacy management desirable.
