The person appointed to the post of Clinical Nurse Specialist (Care of Older Persons) will work within Older Person Services, Mid West Community Health Care as part of a multi-disciplinary team delivering a coordinated approach to patient care with a specific focus on rehabilitation of the older person.

Older Persons Services, Mid West provides services for people in Limerick, Clare and North Tipperary. Services include residential and short stay care, rehabilitation and respite as well as day-hospital services.
The facilities in our Older Persons Residential Units are used in a flexible manner reflecting the service needs at a given time which include: Residential Continuing Care, Residential Rehabilitation, Residential Respite, Short Stay Services and Day Hospitals with associated clinics. Rehabilitation and Medical Day Hospital care is led by a Consultant Geriatrician. The Multi-Disciplinary Team consists of the Medical Team, Nursing Team under direction of ADONs and DON, Physiotherapists, Occupational Therapists, Dietician, Speech and Language Therapist, Multi-Task Attendants and Clerical support.
All service users have the benefit of input from the Multi-Disciplinary Team.
