We are currently recruiting for full-time Director of Clinical Services for Bartra Healthcare.
This is a national post at Executive level which involves the overall management of existing Transitional Care services and the setting up of new services. The role requires extensive knowledge of and coordination with the acute and community healthcare services to facilitate effective and efficient care for patients transitioning between services.
The postholder will liaise closely with the Clinical Director, medical team, Director’s of Nursing and the Allied Health Professional team to ensure all patient outputs and outcomes are measured and analysed to allow for robust governance and compliance.
Responsibilities:
- The further development of the overall vision and strategy for Bartra’s national expansion of transitional care services
- Overall responsibility for the Transitional Care Units to ensure safe and high-quality patient care in line with best practice
- Enhance links with the Executive teams of the relevant acute hospitals to expand and promote Bartra’s Transitional Care Model
- Promote, develop and implement comprehensive transitional care pathways in consultation with the HSE, acute hospitals and community services.
- Manage accreditation and quality assurance initiatives
- Promote the Bartra Transitional Care model through attendance at Conferences and engaging in research.
- Represent Bartra in acute hospital partnerships to promote transitional care excellence.
- Collate, analyse and present relevant data and reports to the Board and HSE
- Identify and implement areas for improvement in transitional care processes.
- Partake and lead on initiatives to oversee the integration of new technologies to enhance care transitions.
Qualifications and Skills
- Bachelor’s or master’s degree in Nursing, Physiotherapy, Occupational Therapy, Medical Social Work, Dietetics, Speech and Language Therapy, Healthcare Administration, or related field.
- Extensive experience and proven leadership within a healthcare setting.
- Familiarity with IT Systems, data analysis
- Experience in budgetary and business models
- Experience in quality improvement initiatives and data analysis.
- Excellent communication and interpersonal skills.
- Strong problem-solving and critical thinking abilities.
- Ability to work collaboratively with and lead a multidisciplinary team.
- Knowledge of healthcare Regulations and compliance standards.
