HCA CIT
A Community Intervention Team CIT team, which has the capacity to deliver personal care to a patient in their own home or a community setting thereby preventing hospital admission or facilitating early discharge. The service hours are 08.00-20.30 over 7 days.
The teams have a skill mix of nurses, health care assistants, occupational therapists, physiotherapists and therapy assistants.
